When will the new Chiswick Health Centre Open?
The health centre will open on 31 March 2025 and patients will be contacted by their practice by text to confirm this.
Which GP surgeries will be moving into the new building?
The practices that moved out in April 2023 will be moving back:

Do patients need to do anything?
Patients registered at the following surgeries;

All patients that are registered with these practices will continue to be able to access their GP at Fishers Lane and will not have to re-register.
Will the process for contacting my GP and booking appointments change?
The current process of contacting your practice and booking appointments will remain the same.
What happens if I have an appointment after the move?
If you have a face-to-face appointment booked after the move you will need to go to the new Chiswick Health Centre on Fisher’s Lane Chiswick. The team will let you know the date of the practice move and look forward to welcoming you to the new centre.
What is the address?
The address is the same as before: Chiswick Health Centre, Fisher’s Lane, Chiswick, W4 1RX
Is there parking?
Pay and display parking bays are available at the rear of the building.
Where will I get my prescription from?
Prescriptions are sent electronically to your nominated pharmacy.
I am not a patient at these practices, but would like to register – what do I do?
New patients can register online – links are on the practice’s website or visit the reception desks for paper registration forms at Stamford Brook or the new site when open. The practices do have catchment areas which are linked to the online form but can also be checked with reception.
What will happen to the health centre at Stamford Brook?
The centre will close when the three GP practices have moved out. There will not be any further patient services on this site.
What other services will be available in the building?
We are working with local community groups and services to ensure that there will be a range of activities and services available throughout the day. More details will follow, for now the GP practices will move back in the spring and this additional offer for the building will follow soon.
